Acknowledgement of Risks
I understand that participation in youth activities involves certain inherent and unforeseen risks.

These risks may include, but are not limited to, falls, slips, trips, collisions, physical exertion, injuries resulting from games or sports, injuries involving recreational equipment, weather-related conditions, transportation-related risks, illness, allergic reactions, food-related illness, contact with other participants, and other accidental injuries or harm.

I understand that some activities may involve risks that cannot be completely eliminated even when reasonable safety precautions and appropriate supervision are provided.

I have had the opportunity to ask questions about the nature of the activities and understand the reasonably foreseeable risks associated with my child's participation.

I voluntarily permit my child to participate in the Church's youth ministry activities and acknowledge that participation involves certain risks.

To the extent permitted by applicable law, I voluntarily assume the risks that are inherent in or ordinarily associated with the activities in which my child participates.

I understand that the Church will take reasonable steps to provide appropriate supervision and maintain a reasonably safe environment, but that no activity or environment can be guaranteed to be completely free from accidents, injury, illness or other risks.

-I am the parent or legal guardian of the youth named on this form;
-I have read and understood this registration and consent form;
-I understand the nature of the regular youth ministry activities;
-I understand that recreational activities involve inherent and reasonably foreseeable risks;
-I have had the opportunity to ask questions about the activities and their risks;
-I voluntarily give my child permission to participate;
-I have provided accurate medical, allergy and emergency information;
-I authorize reasonable emergency medical assistance when necessary;
-I understand that South Shore Church does not provide transportation to or from regular youth group meetings; and
-I agree to the assumption of risk and release provisions in this document to the fullest extent permitted by applicable law.
